Rental of an Elle TENS machine is $75

The rental begins when you are 37wks until your baby has arrived.

Rental includes the use of an ElleTENS machine and one pair of reusable electrode pads.

It is your responsibility to inform us once your baby has arrived so we may pick up the TENS unit from you.

Agreement
1. I do hereby acknowledge and agree that only I, the pregnant person agreeing to this rental contract, am the only person using the TENS machine.
2. I understands that the rental price, $75 for the TENS machine is on a per birth basis of a period of up to 5 weeks with delivery flexibility dependant of other birthing families.
3. I acknowledges that this agreement is non-transferrable.
4. I agree to take full responsibility for the manner in and purpose for which the TENS machine is used.
5. I agree to contact LunaMama Doula Services within two days of baby’s birth to arrange the TENS unit return.
6. I agree to return the TENS unit undamaged following its use.
7. I agree to pay full retail price for the replacement or repair of rented TENS unit in the event that it is damaged, not returned to lessor, or rendered unusable while in my possession.
8. I understand that there will be no reduction in the rental fees if the TENS unit is unused during the rental period, if the TENS unit is returned before the birth or before the end of the rental period.
9. I understand that the rental fee is nonrefundable, and that no portion of the rental fee will be refunded.
10. I understand the cancellation policy and refund policy.
